Finalized Foreign Adoption

The adopting parent(s) who have finalized an adoption of a child in the same country the child was born, and the child now resides in Utah, must supply the following to obtain a birth certificate:

  1. A completed certified Foreign Report of Adoption form.
  2. An application for a New Birth Certificate.
  3. Payment / fees.
  4. Proper identification.

Finalized Utah Adoption

The adopting parent(s) who have finalized the adoption of a child from a foreign country must supply the following to obtain a birth certificate:

  1. A completed certified Report of Adoption form.
    1. A completed certified Order Establishing Facts of Birth - Minor form, or
    2. A completed certified Order Establishing Facts of Birth - Adult form.
  3. An application for a New Birth Certificate.
  4. Payment / fees.
  5. Proper identification.

Citizens Born Abroad

Citizens born abroad are not considered foreign adoptions. You will need to contact the US Department of State Passport Vital Records Section to receive a birth certificate for the birth of a U.S. citizen abroad. More information is available at https://travel.state.gov.
