Two New Parent

A married couple, not related to the child they are adopting, must supply the following to obtain a birth certificate:

1. A certified Report of Adoption form. (This should be sent from the court.)
2. A completed Non-Identifying Health History of the birth parents.
3. An application for a New Birth Certificate.
4. Payment / fees.
5. Proper identification.

Single Parent

A single parent, who may or may not be related to the child they are adopting, must supply the following to obtain a birth certificate:

Single parent with no relation

1. A certified Report of Adoption form. (This should be sent from the court.)
2. A completed Non-Identifying Health History of the birth parents.
3. An application for a New Birth Certificate.
4. Payment / fees.
5. Proper identification.

Single parent who is related

(This is not a stepparent.)
