  1. File the petition for adoption with the right Utah district court


    Contact your local Utah District Court

    Est timeline: 5 min.

  2. Complete a Report of adoption form

    Est timeline: 10 min.

  3. Complete one of the following


    One of the following health history forms (Required if child is under 18 and adoptive parents are not related)

    • Non-identifying health history form OR Health history not available form

    Est timeline: 10 min.

  4. Complete other forms


      Other court-specific forms (varies by district); may include:
      • Consent to adoption
      • Notice of adoption
      • Adoption agreement
      • Affidavit of fees & expenses

    Foreign adoptions finalized outside the U.S.

    If the adoptive child was not born in the U.S. and the adoption was also finalized in a foreign country, you will need to submit a Foreign adoption packet.

    Foreign adoptions finalized in Utah

    If the adoptive child was not born in the U.S. but the adoption was finalized in Utah, you will need to provide both a Report of adoption form and a court order establishing the facts of birth.
